Referrers

A referral is required prior to an appointment being made. Once we have the referral we will contact the patient to organise an appointment. If the patient is a child, please be sure to include the parent’s contact details on the referral.

To make a referral to Dr. Johnson at his Ortho Specialist Centre rooms, please upload your referral below. Alternatively, referrals may be faxed to 03 8699 8554 or e-mailed to admin@osc.com.au

If the matter is urgent, please contact the rooms directly.
